Extended Access

From 1 August 2017, this practice is one of 6 (Farfield Group Practice, Ling House Surgery, North Street Street, Holycroft Surgery, Haworth Group Practice, Kilmeny Surgery), in the Worth Valley taking part in the GP Extended Access Service based here at Farfield Group Practice.  This service will be mean that you may be offered an opportunity to attend an appointment Monday to Friday after 6.30 pm to see a GP or an Advanced Nurse Practitioner.  These appointments are opened on the day itself at 0830 hours and available to request through your own surgery.  The appointment may be with a GP or Advanced Nurse Practitioner and are not to be used for ongoing issues.

As GPs and Nurse Practitioners from all of the six practices involved in this scheme will be working in the Extended Access service at Farfield, your appointment may not be with a clinician from this practice.

Consent to Share

To use the service we will ask for your consent to share your medical records with the Extended Access Service. Without this consent we will not be able to see you in the Extended Access Service as the clinician would not have access to your medical records .  When you are offered an appointment, our receptionists will talk to you about what this means and why it is necessary.

Please click here for information on Consent to Share medical records.
